PIO CARD Question

Hi I got the surrender certificate recently and now in process to apply for PIO Card. Two question:
Do I need to send my original U.S. Passport along with application?
Do I need to send SC certificate original or photocopy is ok?

Assuming you'll be going to the NY consulate (since your N400 was from CT), yes, send your original passport:
