I need to apply PIO Card for my New born baby in USA.We have indian passport with h1 visa.

Which documents is required for PIO card application to Washington Consulate office ?

Any Origional Documents required ( like US passport and US birth certificate ) ?

From where i can notarised the documents ?

I know the Embassy of india website. But on that website it is not clearly mention about origional documents and also which documents need to submit for new born baby in USA.

So that's reason i need answer manually.

please do needful for the same questions.

Can any one tell me if we need to send the empty priority mail envelope with our address written along with the PIO documents?.Can you tell me how many days it will take to get PIO card through mail from NYC office.
