renewal of passport for minor

I m on H4 and my daughter too.
My daughers passport is expiring in 2011 and we r planning to apply for passport renewal. On the consulate site..they asked for 'Nationality status verification form'.. Could u tell me what is this form? and is it really required??

Since when I checked for FAQ it was not mentioned in the documents requirement.
And if I submit the application personally in how many days I can get new passport.

---- The form is essentially used for children born outside Undia (in your case it may the US) to ensure that the child has his/her roots from India and their general antecedeants

----- Filling the form will be of statistical importance and also to get a COI or PIO card. However if at this stage you do not want to complete it at this tme, you can skip it. However, if there are questions by the Indian Consulate it may delay the processing of your Passport

---- If you submit your Passport personnally you should get it back the same day. It could be late in the evening and depending on which Indian Consulate you go to
