US visa c1/D (seaman)

Hi

I am going to get a new US visa , my last one expired 25th 0f August 10.
I am a seaman (master) and we are going to Mexico gulf . What kind og papers a need?. And what kind of application form do I need and where do I find it?.
I am leaving 6th of Oct. so I need to get it at least 5th of Oct. Is that any change to get it before that?
