Passport Expired, Need to Renew

Dear sir.madam,

My Indian passport has expired more than a year ago. I live in the U.S. with a Green Card. From your Website (Immihelp.Com) I filled out the online form for a new passport but it doesn't allow me to get a print out of the form. Instead, it is asking me to submit the form online and take an interview date at the New York Consulate. I just want to send the form by mail. Can you please guide me in the right direction.

Any help will be appreciated much.
