Greencard renewal I-90

i am not sure how to fill ques 3 and 4 of Part 3 in form i-90....i lived in a US city when I applied for my green card.I was a resident of Canada ..I got my consular processing done in Canada ..I'm confused what I should put for the (i) City of residence where I applied for an immigrant visa or adj of status and (ii) consulate where immigrant visa was granted...any help appreciated

From the I-90 instructions:

3. City of residence where you applied for an immigrant visa or adjustment of status - List the location of the U.S. Embassy or consulate or USCIS office where you filed your application for an immigrant visa or adjustment of status.

4. Consulate where immigrant visa was issued or USCIS office where status was adjusted - List location of the U.S. Embassy or consulate or USCIS office where you became a permanent resident

I-90 filing instructions are available on the same USCIS page where the I-90 form is. How difficult can it be to find the information yourself ?
