Hello,
I had applied for mine and my family’s OCI card in March of 2011. I received a letter today stating that my wife’s name in the US passport and Indian passport does not match.
She has her maiden last name in her Indian passport and her married last name in her US passport.
We have been asked to send an affidavit. My questions are
- Where can I get a copy of the affidavit form?
- Where do I need to send it to?
- What other information do I need to send with the affidavit?
- some forums mentioned the need to place ads in US and Indian newspapers for the name change. Is this necessary doe to a last name change due to marriage?
