Questions on PIO App form

Hi,

Me and my wife are Indian Citizens on H1 and H4 respectively. We have a new born and have got him a US passport. We are planning to travel to India and preparing for his PIO card. I have the following Questions on filling the application form

1. For question 23 , where it asks for details of parents/gparents passport details. Should we give both mother and father details or one is enough

2. For Question 24, where it asks for parents/gparents dob details , Should we give both mother and father details or one is enough

3. Is there a need for any original documents from father/mother.
