Documents for COS from H1B to F1 - very urgent

I want to know what documents are required to be sent to for the change of status from H1B to F1 visa?

Please help. Thanks in advance.

Proof of current status - Copies of passport, I-94, petition approval notice, last 3 months paystubs, Visa if any
Supporting docs for future status - Original I-20, admission letter copy (optional), proof of funds available (bank statements), proof of residence in home country (optional)
Add a cover letter explaining why you want to convert to F1.
