need help to re-enter us with greencard after 3 years

Hello:

I need some help on this:
My mother got green card 5 years ago and visited USA 2 times (first 2 years). after that she could not visit usa (health issues in the family) for more than 3 years. out of 5 of her children, 4 of them are here. We never applied for extended stay in India. I went thru web site pretty much. Some say apply for cancellation of green card legally and reapply for visitor visa. I dont think there is an application for finding the status of green card status for reentering. (if stay is more than 3 years).

Please advise how to approach on this issue? Thank you.

She needs to go to the nearest US Embassy/Consulate where she is and check about returning resident visa, I believe there is a form and a couple of documents required to support her case.
