Indian Passport for Children up to 15 years of age (minors)
Each person must have their own separate passport including children. Their name can no longer be included in the parent's passport. Passports for children up to 15 years of age will
be valid for five years from the date of issue or until the child attains the age of 15 years, whichever is less.
If the child is over 14 years but less than 15 years, a normal 5-year validity passport will be issued.
An application for change of appearance is required in cases where there is a significant difference in the appearance of the application from infant to child or from child to adult.
While completing an online application, in the 'service desired' field, choose option 'Passport for Children up to 15 years of age'.

- Notarized copy of child's birth certificate.
- Original passports of both mother and father.
- Attested copies of the first five and last two pages of both mother's and father's passports.
- Consent of BOTH parents (on the last page, page 4) of the online printed application - both
parents should sign and have the signatures notarized;

Submit a sworn affidavit

If only one parent can sign a consent form, that parent should get notarized affidavit explaining why the other parent can not sign the consent form.
