Indian Passport Application in USA - Proof of Residence
You can submit notarized photocopy of any one of the following as the proof of residence:
If you have your parents or other relatives staying with you (not on tourist visa) or your children and there are no utility bills in their name, you should submit your proof of residence, state the name/s of your parent/s (or other relative/s or your children) and state that they are residing in your household. Affix the photographs of your parent/s (or other relative/s) on the photocopy of the proof of residence and have the document notarized. Alternatively, if they are above the age of 60 years, a copy of their State ID may be enclosed.

- State ID
- Landline phone bill displaying the applicant's address
- Water or gas or electricity bill displaying the applicant's address
- House or Apartment Lease Agreement
You don't have to send all the pages of the lease agreement, you just need to send the first page that contains your name and the last page that contains your signature.
- Home Mortgage Bill
- HUD-1 statement if you own a house and don't have mortgage on it
