OCI Miscellaneous Services - Documents
- Application Form:
Print out of the online application form, along with one copy.
Additional requirements: Submit two independent sets of application forms along with all the documents as well, not just two copies of the application form.
- Order Form:
Applicable only for the jurisdictions that process OCI through Travisa Outsourcing:
Complete the online Order Form. You must include a copy of this form with all other supporting documents.

Copy of proof of current nationality:
Additionally, you also need to provide the copy of the information page from the passport (U.S. or passport) that contains your picture and basic details of the passport.
You also have to provide the copy of the endorsement/amendment pages. If you have a 24-page U.S. passport, that would be pages 22, 23, 24. If you have a 28-page U.S. passport, that would be pages 25, 26, 27.
Do NOT send the original non-Indian passport with the application. You will have to send that only after the consulate receives the OCI card from New Delhi, India.
- Copy of naturalization certificate:
If you are former citizen of India or were not born in the U.S., a copy of your naturalization certificate.
In case a minor child acquired U.S. citizenship through naturalization of his/her parents and does not have separate naturalization certificate, copies of the parents' naturalization certificates and a notarized written statement signed by both parents stating the circumstances their child receive his/her naturalization status and why the child doesn't possess his/her own certificate.
If you have obtained more than one nationality, provide all naturalization certificates or proof of nationality from all the countries that you have held citizenship.
- Copy of the surrender certificate.
Applicable only in case of naturalized citizens of Indian origin.
If you don't have it (because there was no such rule when you earlier applied for OCI), you must get it first.

If your name in the U.S. (or foreign) passport is different than the one in your prior Indian passport, additional documents must be submitted.

- Proof of legal status:
If you are a non-U.S. passport holder, you must provide copies of a cancelled Indian passport and the proof of legal status in the U.S., such as a copy of the green card (both sides) or valid non-immigrant status. Non-US passport holder parents applying for their kids must also provide their US visa status. Do not send original documents.

- Minors:
In case you are sending the application for your minor child as an individual, photocopies of parents' OCI cards or U.S. passports & earlier Indian passports.
If applicant's old OCI card was issued before Jan 1, 2010 and if one or both parents are naturalized citizens, submit one of the following as the proof of renunciation of parent(s) Indian citizenship:- Renunciation certificate issued by Consulate of India, or
- Cancelled Indian passport with stamp stating the passport is cancelled due to obtaining US (or any non Indian country) citizenship.

- Re-issuance of OCI documents in case of issuance of new passport, change of
personal particulars, wrong filing of particulars:
Fees in U.S. dollars
In case of applications filed in India, a demand draft for Rs. 1,400/- drawn in favor of "Pay and Accounts Officer (Secretariat), Ministry of Home Affairs" payable at New Delhi.
In case of mistakes committed by the Indian Mission/Post/Office while entering the personal particulars of manually filled applications (now discontinued), no fee shall be charged for re-issuance of OCI documents.

In case of applications filed in India, a demand draft for Rs. 5,500/- drawn in favor of "Pay and Accounts Officer (Secretariat), Ministry of Home Affairs" payable at New Delhi.
When applying in the U.S. or other foreign country, pay by money order or the cashiers check drawn in favor of the Indian Mission/Post/Office you are applying to.
The fees are non-refundable if the requested service(s) are not granted.
- Re-issuance of OCI documents in case of issuance of new passport, change of
personal particulars, wrong filing of particulars:

Enclose- Copy of U-visa pasted on the old passport
and the particular pages of the old passport
Send original old passport as well
In case the old passport is not returned by the government authority while issuing the new passport and copy of the U-visa was not taken prior to the surrender of old passport, it may not be enclosed.
- Original OCI Card
- Up to the completion of 20 years of age, OCI documents will have to be re-issued each time
a new passport is issued.
- Between 21 to 50 years of age, you are not required to seek re-issuance of OCI
documents each time a new passport is issued. However, if you like, you can use this service
for re-issuance of OCI documents.
However, you will have to carry both the old and the new passports while traveling.
- After the completion of 50 years of age, OCI documents will have to be re-issued once
after issuance of a new passport and whenever physical appearance changes.

This could be due to a change by the applicant or wrong filing of particulars by the applicant or Indian Mission/Post/Office.
