  1. Application Form:

    Complete and and print it. Please make sure to answer all questions. Do not leave any answers blank.

  4. Photograph:
    One photograph, to be pasted on the application form. You don't have to provide any additional photographs.

    There is a photo upload option at the Government of India (while filling the application form). Uploading the photo is optional. In any case, one physical (paper) photo is still required.

  5. Copy of current Passport:
    Photocopies of the following pages US passport (or non-U.S. and non-Indian passport for non-U.S. citizens):
    • Information page that contains the picture and basic details of the passport.
    • Endorsement/amendment pages. That is, for 24 pages US passport, copy of pages 22, 23, 24. For 28 pages US passport, copy of pages 25, 26, and 27.

    Do NOT send the original non-Indian passport with the application. You will have to send that only after the consulate receives the OCI card from New Delhi, India. Original passport is required only at the time of OCI match up.

  6. Naturalization certificate:
    Copy of naturalization certificate, as appropriate.

    Enclose proof of payment of the CKGS courier service charges or prepaid envelope with Air Way Bill No # with service provider name.

  10. Proof of Renunciation of Indian Citizenship:
    Renunciation of Indian Citizenship and Surrender Certificate, as appropriate.

    If required, you can just send the renunciation documents and the fees along with the application, instead of first getting the surrender certificate.

  11. Legal status:
    If you are non-US citizen, provide a copy of one of the following:
    Non-US citizens who are applying for their minor children must also provide their US legal status.

    Do not send as original document as they can't be held responsible for loss or damage to original documents.

  12. Minor Applicants:
    1. Parental authorization form. It must be signed and notarized for both parents.

    2. If any of the parents are non US citizens, proof of their legal status in the US, as described above.

    3. Copies of parents US/Foreign passports or OCI cards:
      • US/Foreign passports of parents:
        • Copy of the information page that contains the picture and basic details of the passport.
        • Copy of the endorsement/amendment pages.
      • OCI Cards: If parents have OCI cards, copies of both information pages on the OCI card.

    4. Copies of parents proof of Renunciation:
      If one or both parents are naturalized citizens, proof of renunciation for parents must be provided.

      You should provide either certificate of renunciation issued by an Embassy/Consulate of India or Cancelled Indian Passport with stamp stating the passport is cancelled due to obtaining US citizenship. However, for Washington Jurisdiction, both are required.

      If parents need to obtain renunciation certificate, they need do obtain it prior to submitting application for minor.

  13. Fees:
    1. Re-issuance of OCI documents in case of issuance of new passport, change of personal particulars, wrong filing of particulars:

      Fees in U.S. dollars

      In case of applications filed in India, a demand draft for Rs. 1,400/- drawn in favor of "Pay and Accounts Officer (Secretariat), Ministry of Home Affairs" payable at New Delhi.

      In case of mistakes committed by the Indian Mission/Post/Office while entering the personal particulars of manually filled applications (now discontinued), no fee shall be charged for re-issuance of OCI documents.

    2. Issuance of duplicate OCI documents in case of loss/damage of OCI documents.

      In case of applications filed in India, a demand draft for Rs. 5,500/- drawn in favor of "Pay and Accounts Officer (Secretariat), Ministry of Home Affairs" payable at New Delhi.


    When applying in the U.S. or other foreign country, pay by money order or the cashiers check.

    The fees are non-refundable if the requested service(s) are not granted.

  14. Desired service specific
    • In case of issuance of new passport

      Enclose
      • Original expired US/Non-US passport that contains the U-visa.

        In case the old passport is not returned by the government authority while issuing the new passport and copy of the U-visa was not taken prior to the surrender of old passport, it may not be enclosed.

      • Copy of U-visa pasted on the old passport and the particular pages of the old passport.

      • Original OCI Card, along with copies of registration certificate and information page.

      The OCI registration certificate and U-visa have the passport number of the registered OCI. Whenever a new passport is issued to an OCI holder, rather than going through the inconvenience of re-issuance of the OCI registration certificate and visa each time, as OCI is a life long visa to visit India, the following scheme has been adopted:

      1. Up to the completion of 20 years of age, OCI documents will have to be re-issued each time a new passport is issued.

      2. Between 21 to 50 years of age, you are not required to seek re-issuance of OCI documents each time a new passport is issued. However, if you like, you can use this service for re-issuance of OCI documents. However, you will have to carry both the old and the new passports while traveling.

      3. After the completion of 50 years of age, OCI documents will have to be re-issued once after issuance of a new passport and whenever physical appearance changes.

    • In case of loss or damage of OCI Registration Certificate and/or Visa

      Enclose
      • Copy of the complaint lodged with the Police authorities (in case of loss) or a notarized self-written affidavit explaining the circumstances of loss

      • Original OCI card (in case of damage)


      Personal interview of the applicant shall be taken by the Competent Authority before the grant of the service.

    • In case of change of personal particulars

      This could be due to a change by the applicant or wrong filing of particulars by the applicant or Indian Mission/Post/Office.

      • Original expired passport with U-visa and the copy of the information page of the passport and the U-visa sticker.

      • Original OCI Card

      • The documentary evidence for the change of the personal particulars. E.g., copy of the Nationality certificate (or naturalization certificate) in case of change of nationality.

      Note: In case of change of photograph without the change of the passport, personal interview shall be taken by the Competent Authority before the grant of the service.

    • Correction of OCI Online Form You should select this service if you entered incorrect or erroneous information into the OCI Online Form and would like to submit a correction.
