- Not available if the person is physically located in the state of Maryland, New York, or Washington at the time of application. This restriction applies only to the proposed insureds in New York or Washington, while in Maryland, it additionally applies also to the person applying for the insureds, if they are not the same.
- The following are excluded:
- Treatment of all forms of cancer / neoplasm
- Diseases of the skin
- Diagnosis, treatment, or supplies for the feet; orthopedic shoes or orthopedic prescription devices to be attached to or placed in shoes; treatment of weak, strained, flat, unstable, or unbalanced feet; metatarsalgia or bunions; and treatment of corn, calluses, or toenails.
