Error found in documentation (Middle name missing. This was a software bug on CKGS' side which others have complained about on Yelp. There is no way you can fix this because the filled in form has the middle name but the software generated form does not. Lucky one of their staff understood and they fixed my form, the bug will be fixed some day. I had to pay USPS $8 for a 2 day trackable post for their error).

Pretty good timing considering other posts I saw. What might have helped? Docs were well organized with cover letter. But it may just have been luck!
