Indian Passport

I sent my passport application for Passport reissue with additional of spouse name to CKGS SFO on June 2nd and my courier reached CKGS on June 3rd and received old and new passport today.

Documents Sent

1) Old Passport
2) Money Order issue at Walmart (MoneyGram)
3) Government Online Form Signed on First and last page with Photo
4) Affidavit for Change of Apperance and Signature with Photo - Notarized at UPS
5) Signed Anexure E form
6) Document Checklist
7) US DL(both front and back) and I797 - Notarized at UPS
8) Passport first 7 pages and last 2 pages - self attested
9) Marriage Certificate - self attested
10) Spouse Passport first 2 and last 2 pages - self attested

I opted FEDEX service provided by CKGS.
Pasted the Barcode on FEDEX envelope before sending to CKGS

To track the ‘Progress of Your Shipment’, log onto the site of FedEx and enter your AWB Number.
