How to get multiple entry visitor visa?

I heard that recently, people get 5 years/ 10 years multiple entry visitor visa. How to obtain this type of visitor visa? Will they give for a first time visitor? Is there any special document do we have to attach to request this? I would appreciate if anyone can help me in this. Can I have a sample invitation letter for this type of visitor visa?
