Need example/sample of the statement confirming intent for I485 filing(EB1A)

For EB1A petitioner filing I-485 to change status, there is requirement to submit a signed statement confirming you intend to work in the occupational field specified in the Form I-140.

Anyone has such letter sample or example?

I need proper language and wordings that I can use for my letter.
Any help would be greatly appreciated.
