1. Kindly affix the below label on a cloth lined envelope. Please ensure all documents are 
enclosed as per the checklist below. 
2. Only ONE application per envelope. If you have scheduled your appointment in a group or 
family, please forward multiple envelopes-1 per application. 
3. You can drop in your application at any of the Western India application centres between 
0800 hrs and 1500 hrs or Courier it to us at the below mentioned address. 
4. Kindly ensure that your envelope reaches at a US Visa Application Centre in Ahmedabad, 
Pune or Mumbai latest by minimum of 5 working days prior to your scheduled interview 
date. It is recommended that you forward your documents to us as soon as you schedule 
your Interview. 
5. Please be forewarned that an inability to turn in the application by stipulated deadline wouldresult in cancellation of the appointment.
