I am applying for my childs passport at kozhikode passport office, my husband is presently at vancouver, british colombia, Canada, from the passport office i got the information that i have to get the consent of the father for issuance and that it should be obtained from the indian embassy vancouver may i please have the format of the certificate which my huband should have from the embassy.
