I am applying for my son's PIO who is 2 months old.

In the first question :

1)In the name column is it the complete name or just the first name?
2)There is no Place of issue column in my son's USA passport?What should that be?
3)I got the first 3 pages of my son's USA passport,first 3 pages and last page of mine and my husband's indian passport notarized?
3)Do I have to notarize my son's birth certificate?

I'm not sure bcoz Houston consulate have first name and last name space.

There is something Department of united states mentioned in passport.That you have to write.

Full Name as in First name and last name.

Write "US State Department"

Not required. Only the ppt needs to be notarized when sending by mail.
