Need help reg PIO Card

Hi, I am applying for PIO cards for my husband and my 8-month old son.

The info given on the CGI, Houston website is too confusing. Can you please help me segregating all the documents (as to what all documents to be submitted at the Houston office).

An early reply would be much appreciated.
