Question about Renunciation and OCI

Hello,

While filling the OCI form, One of the very first question that is asked "OCI Govt Reference Number"
What is this reference number and where do i get it from ?

The other questions is with respect to Renunciation. While filling the form, there is a question asking about the Expected Travel Date
I am not expecting to travel anywhere anytime soon. Is it ok to leave it blank?
