Notary Requirements - OCI

We are kind of confused on the application. Following are the documents for an application of a MINOR which we have circled that would require a notary:

• Parental Authorization Form
• Part B of the application

Are there any other documents required to be notarized? We have read post which list

• Affidavit in lieu of originals
• Address proof (Father's document)

Now apart from this question we do need to understand if BOTH the parents would be needed at the notary or just one parent would do especially for Parental Authorization Form? Any experiences?
