Minor OCI - does address proof needs to be notarized?

Hi!

I'm uploading OCI documents for my son, in the checklist it says, "- In case of applicant residing with parents/children/ relatives, a notarized undertaking from them with one of the Notarized copies of address proof as indicated above."

Does the address proof needs to be notarized? If yes, can someone share a template ?
