Iam Planning to take web appointment for my wife and children , can any one send me the Sample DS-156 and DS 157. ( Sample DS-156 which is in the is site is showing for Tourist). i want for spouse and chidren

Actually, the DS-156 instructions are for many different visa types.

You can't expect someone to make samples for every possible scenario and all visa types. That would be thousands of samples. Not practical.

Use the general guideline and also use little bit of your own brain.
