Proof of Health Insurance
Minimum requirements: $250,000 per incident/illness $50 deductible.

That's all they sent to me in an email, all those requirement you listed are not required.

I dont have any pre condiction, the insurance is only a requirment else i dont need one. You can check the school website for details. www.nu.edu
