Need formal budget for Venezuela permission for insurance

I very ineterested to buy an insurance with you (see attachment) however I need a printable (formal) budget to be presented to my goverment. This is because we have some goverment exchange control rules in Venezuela

I would like to buy the insurance (as mentioned in a previous email) but given Venezuela money exchange restrictions I cannot buy it online. However I can pay using my credit card if you send a form for doing that.

There is a paper application available for each plan at https://www.insubuy.com/j1-visa-heal...ce/-insurance/ You can specify the credit card there. Or you can pay with the certified check or money order.
