Address proof required for Native Place entered in OCI application form?

Hi,
I am filing OCI application on behalf of my son who is US citizen.
I would like to provide my address in India as his native address in application form.
but, do we need to submit any proof for that address along with OCI application?
Also, is this address printed somewhere on OCI card?
