Applying OCI for Minor in India by Mother

My son is US born and currently in India with Mother.
I am in US right now. Can my wife alone go to FRRO office and apply OCI for my son?

What other documents need to carry other than US passport, birth certificate, Indian origin proof and Indian visa copies?

Can Mother alone do self attestation of all copies? Any documents need notarization? if yes, who should notarize it?
