i 765 form help needed for L2 EAD

Hi all,

I am on L2 visa and planning to apply for EAD sooner.
can anyone pls answer my queries regarding the i 765 latest form edition 7/17/17.

1) what shall be the preference while filling the form i 765? write or type?
2) if i say yes to item number10. i am unable to type in the fields 11 to 13. I am not sure if i can do both like type and write both at the same time on the form. pls help
3)same happened while filing the form g1145. i am unable to type my number in the form.so is it okay if i just write that field with pen and type in the rest of the fields?

list of documents that i am attaching are:
check
g1145
i 765
i 94 record of both mine and my spouse
copy of passport - mine and my spouse
marriage certificate copy
two photo

4) Please let me know if i shall send some other document as well.

yes it is turned on. but still i am unable to type. dunno what do to.
Is there anyone here who has fiiled the form by writing instead of typing?? pls do reply.
