EAD renewal Help!

Hi Guys!

I had been reviewing posts but I couldn't get a clear answer how I can go through with EAD renewal

so questions if you may:

1. What is the process for the renewal of EAD combo card
2. Do I submit I131 and I765 again and send it together in one package?
3. What documents do I need?

I-765/EAD Renewal
? Completed form I-765, Application for Employment Authorization;
? 2 recent color photographs (A# and Name lightly printed in the back);
? Copy of previously approved EAD/AP Combo Card (Front and Back);
? Copy of previously approved I-765/EAD Approval Notice;
? Previously approved EAD mailer information cut-off section;
? Copy of I-797C, Notice of Action, receipt of filing I-485;
? Copy of Passport?s biographic page;
? Copy of Driver's License(if u have one)
? Form G1145 for e-notification for application acceptance
