OPT Extension: forgot to update end of employment date

So, I am on OPT Extension. I finished working with company A in Apr, did final evaluation with I983, but forgot to send it to the DSO. Now it is June and I just start working with company B. Could I still send two forms: I983 (of company A) and I983 (of company B) to the DSO? Is there any problem? Thanks!
