help with vcu1 form for usa visa

hi. i need a bit of information required for the vcu1 form, its asking have you been previously been found ineligible for a us visa please state the the section of law under which where found ineligible if kown. like i have previously been granted a visa the last time i applied but cant remmember what to write in the box and as to did you receive a waiver should i say yes
