Medical Visa denied applying again

Hi, I applied for a medical visa and I got denied. I'm applying again with the same evidences since I think it is unfair and I desesperately need treatment, but I dont remember the exact dates I put on my previous jobs in q.12 on DS 157 (dd/mm/yy I only remember month and year for my lasta emplyee)

Is this a problem? can I fill in only month and year?

What country are you from? And what kind of medical treatment do you need?

I need treatment for a chronic dissease of the bladder. Doctors are not familiar with it in my country and the medicines are not available here. I'm from Ecuador. Is there any problem with the dates on the DS 157?
