urgent help needed

I am a physician on B1, came to the US to complete medical exams and now I have been offered a research position by a teaching hospital on a H1 visa.

should i get H1 papers here itself or should i go back to my home country and apply with my consulate. I am confused which is better

My I 94 is good for another 2 months.
