D-160, Abbreviation (initials) of surname in academic records

  • D-160, Abbreviation (initials) of surname in academic records

    I have the following doubt while filling the D-160 form.

    My complete name is:
    First Name: Sethuraj
    Surname: Karimparambil Raju

    My passport also has the same name. But in most of my academic certificates, the Surname is abbreviated to initials, and hence the name is written as

    Sethuraj K. R. or Sethuraj K R

    Do I need to mention this in the "other name" section of the D-160 or I can ignore it as it seems to be very obviously the same name?

    Yes, mention the name used in your academics as "other names" in DS160.

    Also, be ready to prove that both are your names. For example, academic marksheets showing your DOB matching your DOB in passport.
