j1 to h1 transfer

I have read restrictions on http://uscis.gov/graphics/howdoi/exchvisit.htm.

Wondering how easy it is to convert from j1 to H1,

1. If same employer who sponsored j1 agress to convert to h1
2. Can a j1 trainee change the employer?

I am not medical graduate or trainee, I think most of the limitations are surrounding medical trainee or graduate or if j1 is sponsored by US govt or gov activity.
