Difference between New H1B stamping and extension H1B stamping

Hi,

While filling up DS160, there is no where we mention that we are going to appear for stamping interview first time or this is the second(or more) stamping interview after extension.

My question is how would interviewer come to know that I was already staying in US and have gone to India for a month and appearing for stamping because of extension?

Do I need to tell interviewer specifically or there is any other way?

First of all there are questions about previous Visa issuances and past travels.

Technically, every Visa application is a new application. It makes no difference if you have previously been to the US. In case you have a previous US Visa of the same class, the consulate sometimes waives some of its requirements/procedures, but it still is a new Visa application.
