Last name change in H1B visa, I-94 and other documents

I am currently divorced and want to change my name in the Indian passport and all US documents. I came here on F1 visa (with married name), and recently got my H1B visa stamped (married name). Since my passport with married last name is expiring in August 2013, I am planning to apply for passport renewal and name change together, followed by name changes in US visa and other documents (I-94, SSN, driver's license, etc.). What is the procedure to apply for US visa name change?
Secondly, since my passport is expiring soon, my I-94 has been stamped with August date. What should I do in this case?

Were you able to have your Lastname changed on the Visa and I-94. I am trying to get this done but I am unable to find a proper post that could spell it out clearly.... Do share your experience
