Passport requirements for new born child

Hi,

I am travelling on a h1b visa. I am planning to take my wife and my new born child along with me. Do we require a passport for the child or it is ok for the name to be included in mother's passport?

No. You will require new passport for the child. And also visa application forms and fees.
