H1B and H4 Stamping need Urgent Help.

Hi,
My petition for H1B got approved but i was not given I-94 because when i applied MY I-94 was expired.So is the case with My Husband who is on H4.When we applied for his extension along with mine his got rejected because my extension took time as i had got RFE and i had to reply for that RFE and it took time.Now,My petition is approved....i want to know
1.what is his status?
2.I was filling the visa appointment date in that they are asking for mine and his petition number.What should i give his petition number?Please suggest I need to book visa date ASAP.

Both of your current location,petition filing dates (Receipt dates), petition expiry dates, I-94 expiry dates for both of you.
