H4 dependent visa

I am currently working in USA, I have H1 B Visa. I have few query for H4 dependent visa before going to USA consulate.

1. My spouse dose not have surname. (In the passport surname field is blank)

2. In the passport husband section is blank, because she applied the passport before our marriage.

Please let me know shall we go for H4 dependent visa processing.

Marriage certificate as well as marriage photos, and proof of you maintaining valid H1 status is important.
