Questions about H-4 visa

Hi,

My wife needs to apply for a dependent H-4 visa. In regards to that I’ve 2 questions –

1. Her current state of residence is in different US consulate jurisdiction (Mumbai) from mine (New Delhi). So, I’m wondering which US consulate in India she needs to take the visa appointment with after marriage?

2. Her last name is different from mine; does she need to get it changed in her existing passport? Also, is it recommended to have my name as the spouse in her passport before going for the visa?

As per my knowledge your name should be added in the passport as spouse, changing family name is an option, its not really necessary, you guyz can decide on it. She falls into the mumbai area as her proof of residence (PASSPORT) says so.
