H1b extension from India for expired visa

My h1b petition and visa both are expired on 30 sep 2014 and I am back to India.
My i-94 has also expired on 10th oct 2014.

Can my employer file cap exempt visa extension from India. If yes do I have to appear for visa interview again or only stamping is needed.

I was in US from dec 2011 till sep 2014.spent almost 3 years
