What to do when H4 expires

Hi All,
I am currently on H1b and My wife is on H4. I renewed my H1 at 09/14 and got new H1 till 09/16. But I didn't realized that I had to also renew H4 for my wife. And her H4 got expired at 09/14. So currently she has expired H4.

So what is next for me ?
Can I just apply for her H4 renewal while she's in USA ? Or she needs to go back to India until it gets renewed ?
