My Current H1B valid until: 05/11/2015

Today Extension Approved From: 05/11/2015 to 05/10/2018

Current Visa stamping expired on 10/11/2010

I am going to India on Vacation for a month 01/FEB/2015 to 01/MARCH/2015

I am filling DS-160 Form:

1. Which H1b Petition I should use in my case? New or Old petition
2. On the Visa what will be the issue date and expiry date in case my visa gets approved?
3. And is there any issue on the Port of Entry?
