Is it possible to apply for extension beyond the term?

Are you still employed? How long have you been out of work without pay slips? Do you have medical certificates to support your health issues that prevented you from working and getting paid while on H1B?

Not unless you have an approved I-140 or a pending PERM that is filed 365 days prior to your end of the 6th year on H1B.
