i-94 extension for h4 visa

I am on H1b visa which is valid until 2007 may(i-94 valid until 2007 may). My wife and my daughter also has the H4 visa until 2007, but the i-94 for my daughter expires on july 11 2006 because of her passport expiry. she got her new passport recently.Could you pleas advise me ,
1. how to apply for i-94 extension?
2. when should i apply for the extension?
3. what are the documents I should attach with the application?

you can apply for the H4 extension ( only till the validity of your H1 visa only) by filing I-539 with new passport and copies of your H1- 797,i-94,passport with visa stamping and a fee of $200/-
