H1 to h4 cos and h4 denied for my son

Could you please let me know next steps ?

My son's H4 is on my wifes h1 petition which is valid till march 2018.

We have applied for my wife H1 to H4 and H4 extension (in single application) for my son on my H1B petition. My wife's H4 is approved but my son's H4 extension denied(reason is cannot apply in single application)

So is he out of status now(since he has valid h4 approval on my wife's h1, but my wife is h4 now)? What are the next steps? Can I reapply H4 extention on my h1b approval?
